Thank you for considering CORE Veterinary Rehab for your patient's rehabilitation needs.

In compliance with Wisconsin state regulations and, in an effort to keep an open line of communication with the primary veterinarian, we require all clients to obtain a veterinary referral for their pet prior to receiving rehabilitation services at our facility.

2. Pertinent medical records and images

3. Proof of current rabies vaccination or protective rabies titer
